Job summary

Service Access & Management, Inc. (SAM) is seeking a Supports Coordinator for the IDD service line in Lewistown, PA. You will provide person-centered case management, advocacy, and coordination of services for individuals with IDD.

Starting rate is $21.00 per hour. You will assess goals, develop plans, coordinate supports, monitor delivery, and document activities to meet regulatory standards. Qualifications include a bachelor’s degree with social science credits (or 2 years related experience),

Qualifications

  • Bachelor’s degree with at least 12 credits in social sciences; or 12 social-science credits plus 2 years in human services.
  • Ability to manage a dynamic caseload independently.
  • Strong interpersonal, organizational, and communication skills.
  • Valid driver’s license and reliable transportation.

Responsibilities

  • Assess strengths, needs, and life goals.
  • Develop person-centered plans with the individual and circle of support.
  • Coordinate services to align with goals and ensure timely access.
  • Monitor service delivery and adjust supports as needed.

About The Supports Coordinator - Case Manager Role

As a Supports Coordinator within our Intellectual and Developmental Disabilities (IDD) service line, you’ll play a vital role in supporting individuals with IDD, including those with Autism, in living self-directed, fulfilling lives. You’ll build long-term relationships through ongoing case management, advocacy, and coordination of meaningful services. This position is ideal for someone who is compassionate, detail-oriented, and thrives in community-based environments where collaboration and relationships matter.

Starting Rate: $21.00/hourly

Supports Coordinator - Case Manager Core Responsibilities
  • Assess individual strengths, needs, and life goals
  • Develop person-centered plans in collaboration with the individual and their circle of support
  • Coordinate services that align with individuals’ goals and ensure timely access to supports
  • Monitor service delivery and adjust supports to remain responsive and effective
  • Document all activities and communications according to regulatory and agency standards

What You Bring
  • Bachelor’s degree with at least 12 credits in social sciences (e.g., social work, psychology, sociology, criminal justice); OR
  • A minimum of 12 credits in social sciences plus 2 years of related experience in human services
  • Ability to work independently and manage a dynamic caseload
  • Strong interpersonal, organizational, and communication skills
  • A valid driver’s license and reliable transportation

Location & Schedule

This position is based in Lewistown, PA. Work is primarily community-based with remote flexibility.

Office hours are Monday through Friday, 8:30 AM to 4:30 PM.
