Job summary

Pathways Forward in Harrisburg, PA is seeking a Supports Coordinator for the Intellectual and Developmental Disability Program. This full-time, day-shift role focuses on creating person-centered support plans and coordinating services for individuals and families.

Relocation to Harrisburg is required before starting. The position offers paid holidays, personal days, vacation, medical and retirement benefits, and eligibility for incentives for Master’s degrees or bilingual/ASL skills.

Qualifications

• Bachelor’s degree with at least 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or related social science.
• Associate degree with at least 12 college credits in the above fields and two years of relevant experience.
• Clearances: PA Child Abuse, PA Criminal History, and FBI.
• Valid driver's license and reliable vehicle.

Responsibilities

• Facilitate the creation of person-centered support plans with individuals, families, and support teams.
• Identify and refer community and system supports and services to meet individual needs.
• Coordinate and monitor the delivery of supports and services as outlined in support plans.
• Ensure the quality, type, duration, and frequency of services meet individual needs.
• Monitor health and wellbeing of participants through regular contact with individuals, families, and service providers.
• Identify unmet needs and document them for future planning and funding opportunities.
• Inform individuals of their rights and maintain comprehensive documentation through a statewide web-based system.
• Participate in ongoing training to enhance your skills and knowledge.

Education

Bachelor’s degree + 12 credits in social sciences

Associate degree + 2 years relevant experience
