Job summary

The Arc Alliance seeks a dedicated Supports Coordinator (Exempt Staff) to assist individuals with ID/DD across multiple PA counties. This role requires travel to participants' homes and community locations to coordinate services and monitor health and safety under Medicaid waiver programs.

Responsibilities include developing ISPs, coordinating team meetings, ensuring ISP compliance, and maintaining thorough documentation. A relevant degree and clearances are required; travel is essential.

Qualifications

  • Bachelor's degree from an accredited college or university.
  • Minimum of 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or related social sciences.
  • Current FBI, Criminal History, and Child Abuse Clearances (required at hire and maintained per regulations)
  • Valid Pennsylvania Driver's License
  • Current automobile insurance (recommended coverage: $100,000/$300,000 liability and $100,000 property damage)
  • 3-year driving record check (updated annually)
  • Willingness and ability to travel throughout service counties

  • Facilitate annual ISP reviews and reassessments
  • Maintain documentation in HCSIS
  • Support individuals and families throughout the planning and service process
  • Provide information on fair hearing rights and assist with requests
  • Participate in community outreach and agency fundraising initiatives
  • Complete all required trainings

We are currently seeking dedicated and compassionate Supports Coordinators to provide case management services to individuals of all ages with intellectual and developmental disabilities (ID/DD). This role supports individuals living in community-based residential programs or in their own/family homes across Berks, Bucks, Chester, Delaware, Montgomery, and Philadelphia Counties.

As a Supports Coordinator, you will play a critical role in monitoring health and safety, coordinating services through Pennsylvania's Medicaid waiver programs, and connecting individuals and families to vital community and natural resources.

Position Summary

Under the direction of the Director of Supports Coordination Services, the Supports Coordinator is responsible for:

  • Providing comprehensive case management services
  • Developing and maintaining Individual Support Plans (ISPs) using a person-centered planning approach
  • Monitoring participant health, safety, and service delivery
  • Coordinating and facilitating team meetings
  • Ensuring compliance with all state and agency regulations
