Job summary

Service Access & Management, Inc. (SAM) seeks a Supports Coordinator in the Intellectual and Developmental Disabilities service line to help individuals with IDD live self-directed, fulfilling lives.

You’ll build relationships through case management, advocacy, and coordination of services in a community-based setting. This full-time role in Lebanon County, PA offers a starting rate of $20 per hour, with opportunities for professional growth and ongoing training.

Qualifications

• Bachelor’s degree with at least 12 credits in social sciences; or 12 social science credits + 2 years related human services experience.
• Ability to work independently and manage a dynamic caseload.
• Strong interpersonal, organizational, and communication skills.
• Valid driver’s license and reliable transportation.

Responsibilities

• Assess individual strengths, needs, and life goals.
• Develop person-centered plans with the individual and circle of support.
• Coordinate services aligned with goals and ensure timely access to supports.
• Monitor service delivery and adjust supports as needed.
• Document activities and communications per regulatory and agency standards.

About The Supports Coordinator - Case Manager Role

As a Supports Coordinator within our Intellectual and Developmental Disabilities (IDD) service line, you\'ll play a vital role in supporting individuals with IDD, including those with Autism, in living self-directed, fulfilling lives. You’ll build long-term relationships through ongoing case management, advocacy, and coordination of meaningful services. This position is ideal for someone who is compassionate, detail-oriented, and thrives in community-based environments where collaboration and relationships matter.

Starting Rate: $20/hour

Supports Coordinator - Case Manager Core Responsibilities

• Assess individual strengths, needs, and life goals

What You Bring

• Bachelor’s degree with at least 12 credits in social sciences (e.g., social work, psychology, sociology, criminal justice); OR
• A minimum of 12 credits in social sciences plus 2 years of related experience in human services
• Ability to work independently and manage a dynamic caseload
• Strong interpersonal, organizational, and communication skills
• A valid driver’s license and reliable transportation

Location & Schedule

This position is based in Lebanon County, PA. Work is primarily community-based with remote flexibility.

Office hours are Monday through Friday, 8:30 AM to 4:30 PM.
