Job summary

Partnership for Community Supports seeks an Intellectual Disabilities Supports Coordinator to provide supports coordination for individuals with intellectual disabilities and autism. Duties include developing person-centered plans, locating, coordinating and monitoring services, and ensuring health and safety for clients on caseload.

You will enter Individual Support Plans into the Home and Community Services Information System, facilitate team meetings, and connect clients with community

Qualifications

  • Bachelor's degree or equivalent combination including 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or related field and experience.
  • Two years of college coursework in related social sciences plus experience as a County Social Services Aide 3 or similar paraprofessional case management.
  • Local travel required with valid driver’s license; mileage reimbursement provided.
  • Excellent computer, verbal and written communication and organizational skills.
  • Experience with individuals with disabilities preferred.
  • Bilingual ability is a plus.
  • Access to personal secure home WiFi is preferred.

Job description

Intellectual Disabilities Supports Coordinators provide supports coordination for individuals who have intellectual disabilities and autism and their families on the phone and in-person, performing home and program visits. Supports Coordinators ensure that the needs and wishes of the individuals are addressed and life opportunities are expanded.

Duties include: Developing person-centered plans, locating, coordinating and monitoring services and family driven supports for individuals with intellectual disabilities or autism. Monitoring the health and safety for individuals on assigned caseload.

  • Develop and enter Individual Support Plans into the Home and Community Services Information System within required time frames, assuring the active involvement of the individual receiving service and his/her team, and in keeping with all Federal, State, and City compliance requirements.

Requirements

  • A Bachelor’s degree, which includes or is supplemented by at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social science; or two years’ experience as a County Social Service Aide 3 and two years of college level course work, which include at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social service; or any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social service and one year of experience as a County Social Services Aide 3 or similar position performing paraprofessional case management functions; or a combination of 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or other related social science and two years’ professional experience in developmental disabilities, special education, mental health, counseling psychology, school psychology, social work or health and rehabilitative services.
  • As position includes local travel, a valid driver’s license and use of personal automobile are required to complete home visits and program visits. Mileage reimbursement currently $0.67 per mile.
  • Excellent computer, verbal and written communication and organizational skills needed.
  • Job requires a sensitive, caring individual who wants to make a difference in the lives of individuals with intellectual disabilities and their families.
  • Some experience working with individuals who have disabilities is preferred.
  • Bilingual ability would be an asset.
  • Access to a personal, secure, password protected home WiFi connection is preferred.
