We are seeking a Supports Coordinator.This role isresponsible for Person-Centered planning to develop an Individual Plan of Service. The Supports Coordinator will provide on-going linking, coordinating and monitoring of services.
This is a full-time, hybrid position, working Monday to Friday from 8:00am to 5:00pm. While the position is remote, the employee in this role will meet with clients in the Kent County community as part of their responsibilities.
