Job summary

STEP is hiring a Supports Coordinator in Wayne County, MI. You will connect individuals with needed services, create person-centered plans, and coordinate resources to help people live more independently.

As part of the Clinical Team, you’ll develop vocational profiles and employment plans, monitor progress, maintain records, and ensure compliance with consents and requirements. This is a full-time, in-person role offering a mission-driven environment and growth opportunities.

Qualifications

• LSST/SST/LLBSW/LBSW/LLPC/LLP/LMSW/MSW licensure required.
• QDIP and QMHP credentials required.
• Bachelor's Degree in Human Services (social work, psychology, etc.) preferred.

Responsibilities

• Create and oversee person-centered plans to meet each individual’s goals.
• Identify needs, dreams, and aspirations during the planning process.
• Link and coordinate resources, supports, and services.
• Collaborate on vocational profiles and employment plans.
• Monitor, review, and adjust supports as needed.
• Maintain accurate clinical case records and documentation.
• Ensure compliance with consents, physicals, and other requirements.

We're looking for a passionate and dedicated Supports Coordinator to join our Clinical Team and help the people we serve reach their fullest potential.

What You'll Do

Qualifications:

• Licensure as LSST, SST, LLBSW, LBSW, LLPC, LLP, LMSW, or MSW
• QDIP and QMHP credentials
• OR a Bachelors Degree in Human Services (social work, psychology, etc.)

Schedule

• 7.5-hour shifts, Monday-Friday
• Must be able to commute to and throughout Wayne County, MI
