Qualifications

• Bachelors in social work, sociology, psychology, or related field.
• Master's degree preferred.
• Minimum of two years' experience with formerly homeless or similar populations.
• Ability to work independently and within an interdisciplinary team.
• Strong organizational skills and documentation discipline.
• Experience with electronic case management systems (ETO or similar).

Responsibilities

• Provide integrated counseling and case management services to engage residents and promote housing stability.
• Develop client-centered, recovery-oriented service plans with short and long-term goals.
• Use a harm reduction approach with mental health and substance-use needs.
• Make referrals and liaise with care coordination agencies and health providers.
• Engage residents through apartment visits, community activities, and appointments.
• Develop safety plans and de-escalate crises with the team.
• Develop and maintain confidential records per schedule.
• Collaborate with Leasing and Compliance to ensure housing stability.
• Participate in staff meetings and ongoing clinical supervision and trainings.
