• Experience with ETO or other electronic case management systems.

Responsibilities

• Collaborate with residents to develop client-centered, recovery-oriented service plans with short- and long-term goals.
• Use a harm reduction approach to support mental health and substance use challenges.
• Make referrals and liaise with care coordination agencies and health providers to secure services.
• Develop safety plans and participate in team meetings and trainings to ensure best practices.
