Job summary

Qualifications

• Bachelor's degree from an accredited institution required, preferably in social work, sociology, or psychology.
• Master's degree preferred.

Responsibilities

• Provide integrated counseling and case management services to residents to promote health, stability, and housing.
• Collaborate with residents to develop client-centered, recovery-oriented service plans with short- and long-term goals.
• Utilize a harm reduction approach to support a diverse caseload around mental health and substance use.
• Make referrals and liaise with care coordination, health, mental health, substance use, and employment services.
• Engage residents through regular visits, community activities, and escort to appointments as needed.
• Develop safety plans and collaborate to de-escalate crises as they arise.
• Assist in developing and implementing groups/programs to meet residents' needs and foster an inclusive community.
• Maintain confidential resident records documenting interventions and communications per schedules.
• Collaborate with Leasing and Compliance to ensure housing stability.
• Participate in staff and clinical meetings and ongoing supervision.
