Job summary

Lantern Community Services, a Bronx-based nonprofit, seeks a dynamic Case Manager to support individuals with chronic medical conditions and housing stability. You will assess, plan, link to medical and social services, and document progress within an interdisciplinary team.

Qualifications

  • Bachelor’s degree (BA/BS) from an accredited institution required.
  • Master of Social Work (MSW) strongly preferred.
  • Minimum of one year of experience working with adults/families in poverty, chronic homelessness, mental illness, substances use, or medical issues.
  • Ability to work independently and as part of an interdisciplinary team.

About The Role

We are seeking a dynamic and experienced Case Manager to support individuals with chronic medical conditions, including HIV and AIDS, in the community. The Case Manager is part of an interdisciplinary team focused on helping clients stabilize their lives, maintain housing stability, and work toward their physical, emotional, vocational, educational, and social goals.

This role includes conducting comprehensive assessments, developing and implementing individualized service plans, linking clients to medical, mental health, substance use, and social service providers, monitoring client progress, and maintaining accurate documentation. The ideal candidate brings strong clinical knowledge and experience working with individuals and/or families impacted by mental illness, substance use, medical issues, and involvement with legal or criminal justice systems. The successful candidate is compassionate, proactive, and resourceful, and is able to work effectively both independently and as part of a collaborative team.

Essential Functions

  • Provide integrated counseling and case management services that effectively engage residents and promote health and wellness, personal growth, and housing stability.
  • Collaborate with residents to develop client-centered, recovery-oriented service plans that identify both short- and long-term goals.
  • Utilize a harm-reduction approach to support a diverse caseload of formerly homeless individuals experiencing mental health and/or substance use challenges.
  • Make referrals to, and serve as a liaison with, care coordination agencies, health and mental health providers, substance use programs, and employment services to secure appropriate supports.
  • Actively engage residents through regular apartment visits, community outings, and accompaniment to appointments, as needed.
  • Develop safety plans for residents and collaborate with the team to de‑escalate crises and manage challenging situations as they arise.
  • Develop and facilitate groups and programs that are responsive to residents’ needs and foster an inclusive, respectful community environment.
  • Maintain accurate, confidential records for each resident in accordance with mandated documentation schedules, including assessments, interventions, and correspondence with collateral providers.
  • Collaborate with the Leasing and Compliance teams to support and maintain housing stability.
  • Participate in staff meetings, clinical meetings, and individual supervision to ensure best practices in service delivery.
  • Participate in group clinical supervision toward advanced licensure.
  • Engage in a robust training program, including training in evidence‑based practices, to ensure the highest quality of services for clients.
  • Perform other job-related duties as assigned.
Required Education And Experience
  • Bachelor’s degree (BA/BS) from an accredited institution required; Master of Social Work (MSW) strongly preferred.
  • Bilingual Spanish/English preferred.
  • Minimum of one year of experience working with adults and/or families impacted by poverty, chronic homelessness, mental illness, substance use, and/or medical issues.
  • Ability to work independently as well as collaboratively as part of an interdisciplinary team.
  • Strong organizational skills with the ability to manage a diverse caseload of clients with diverse needs.
  • Ability to work effectively under pressure, including in crisis situations, and demonstrate flexibility with work schedules when necessary.
  • Experience using Case Management electronic record‑keeping systems.

Work Environment: This job operates in a supportive housing environment and works with a population who may experience mental illness, substance use, disability, HIV/AIDS, and other chronic illnesses.

Physical Demands

While performing the duties of this job, the employee is regularly required to stand, walk, sit, stoop, climb stairs, and/or kneel. Occasionally, the employee will have to lift and/or move up to 10 pounds.
