Job summary

Lantern Community Services, a Bronx-based nonprofit, seeks a dynamic Case Manager to support individuals with chronic medical conditions and housing stability. You will assess, plan, link to medical and social services, and document progress within an interdisciplinary team.

The role emphasizes compassionate, proactive work with residents, including those affected by HIV/AIDS, mental illness, and substance use, to help them achieve physical, emotional, vocational, educational, and social goals.

Qualifications

  • Bachelor’s degree (BA/BS) from an accredited institution required.
  • Master of Social Work (MSW) strongly preferred.
  • Minimum of one year of experience working with adults/families in poverty, chronic homelessness, mental illness, substances use, or medical issues.
  • Ability to work independently and as part of an interdisciplinary team.

Responsibilities

  • Provide integrated counseling and case management services to engage residents and promote housing stability.
  • Collaborate with residents to develop client-centered, recovery-oriented service plans with short- and long-term goals.
  • Utilize a harm-reduction approach for a diverse caseload with mental health and substance use challenges.
  • Link clients to medical, mental health, and social services and coordinate with providers.
  • Maintain confidential records and documentation in accordance with schedules.
