Job summary

Lantern Community Services, Inc. is seeking a Case Manager on a full-time basis to support formerly homeless adults in a supportive housing setting. You will provide counseling, coordinate care, and help residents stabilize housing and overall well-being.

Qualifications

• Bachelor’s degree required; Master’s preferred in related field.
• Two years’ experience with formerly homeless adults or similar populations.
• Ability to work independently and in an interdisciplinary team.
• Experience with electronic case management systems.

Responsibilities

• Provide integrated counseling and case management to residents.
• Develop client-centered, recovery-focused service plans with goals.
• Use harm-reduction approaches for mental health and substance use.
• Refer to health, housing, and employment services as needed.
• Document interventions and coordinate with collateral services.
• Participate in staff meetings and supervision; support crisis response.
