Job summary

Chicanos Por La Causa, Inc. (CPLC) LUCES team is hiring a Supportive Case Manager to assist people living with or at risk of HIV/AIDS.

This bilingual English/Spanish role involves direct client support, needs assessments, treatment plan implementation, referrals, and coordination with medical and community teams across Maricopa and Pinal counties. You'll engage clients in offices, homes, and community settings, advocating with empathy and non-judgmental support.

Qualifications

  • Bachelor's degree in a licensed field related to case management (social work, nursing, public health or other human services)
  • Or high school diploma plus 4 years of comparable professional knowledge and abilities in case management (needs assessments, treatment plans, referrals)
  • Knowledge of HIV/AIDS and ability to educate clients about HIV/AIDS and STDs
  • Bilingual: English/Spanish skills
  • Valid driver's license and automobile insurance

Responsibilities

  • Provide direct client services and coordinate with community supports to manage an assigned caseload
  • Conduct needs assessments and implement treatment plans; facilitate referrals
  • Manage clinical care team involvement and participate in ongoing learning experiences
  • Engage clients in a variety of settings including office, home, and offsite locations across Maricopa and Pinal Counties

The CPLC LUCES team is hiring a Supportive Case Manager to support those living with, or at-risk of HIV/AIDS.
  • Note this role is a bilingual Spanish/English-speaking role in order to best support our clients.
About Cplc Luces

CPLC LUCES (Latinos Unidos Contra El Sida/Latinos United Against Aids) provides HIV prevention education and supportive services to people living with or at risk of HIV/AIDS to improve health outcomes, reduce health disparities, and address barriers to treatment.

WHAT IMPACT WILL YOU MAKE?

You will break down barriers for those with, or at risk of, HIV/AIDS, by providing direct covered services to individual clients and coordinating with community support and stakeholders. As the Supportive Case Manager, you will have an assigned caseload, manage referrals to medical services, conduct needs assessments, implement treatment plans, facilitate referrals, manage clinical care team, and participate in ongoing learning experiences.

You know the drill! This is a community-based role that will send you throughout Maricopa and Pinal County. You are going to engage with clients in different settings, including, but not limited to, the office, homes, offsite community and/or clinical locations and primary medical care appointments as needed.

  • You're on a mission to reshape perspectives! You model non-judgmental, unconditional support to communities who historically face barriers and stigma in accessing care, particularly Spanish-speaking and LGTBQIA2+ communities.
  • You have the knowledge of HIV and health! You understand the dynamics of HIV, family violence, substance abuse and the stigmas facing under-served communities.
  • You have the skills to find resources! Your experience with psychosocial assessments, the Ryan White healthcare system, navigating community resources and supporting clients in securing services makes you an asset the LUCES team.
Minimum Qualifications
  • Bachelor's degree from an accredited college in a licensed* field related to case management such as social work, nursing, public health or other human services related field.
  • Or high school diploma plus 4 years of comparable professional knowledge, skills and abilities specific to supportive case management (conducting needs assessments, implementing treatment plans, facilitating referrals) may be substituted for the degree.
  • Knowledge of HIV/AIDS and skilled at educating clients about HIV/AIDS and other STD's.
  • Bilingual: English/Spanish skills.
  • Valid Drivers license and automobile insurance.
