Job summary

Goodwill of the Heartland is seeking a Supported Community Living Associate in Iowa City to build relationships with participants and connect them with community resources.

The role involves assessing needs, monitoring progress, managing expenses, writing case notes, and coordinating ISP development. Schedule is Monday–Friday, 8:00am–5:00pm, with travel between locations required, and a starting wage of $16.78 per hour.

Qualifications

• High school diploma or GED required.
• Six months experience in training and/or rehabilitation required; post-secondary education may be substituted for experience.

Responsibilities

• Develop and maintain effective relationships with clients.
• Identify and maintain current information about community resources that may benefit program participants.
• Identify treatment and service needs of each individual served.
• Monitor and evaluate progress of the treatment plan, assisting and facilitating as appropriate.
• Monitor and control expenses and ensure efficiency in rehabilitation services, including billable hours and client funding.
• Write quality case notes and assist SCL Specialist with ISP Development and Progress Reports.
• Provide transportation to participants as needed.
• Have access to a safe, reliable vehicle.
