Job summary

Goodwill of the Heartland is seeking a Supported Community Living Associate in Cedar Rapids, IA. The role focuses on developing strong client relationships, coordinating with community resources, and documenting progress and service plans.

Schedule is Monday–Friday, 8:00am–5:00pm, with travel between participant locations. The starting wage is $16.78 per hour, with benefits including PTO, insurance options, a retirement package after one year, and a store discount.

Qualifications

• High school diploma or GED required.
• Six months experience in training and/or rehabilitation required.
• Reliable vehicle and valid Iowa driver's license; proof of auto-insurance; background checks completed.

Responsibilities

• Develop and maintain relationships with clients.
• Identify and utilize community resources benefiting participants.
• Identify treatment and service needs of each individual served.
• Monitor progress of treatment plans and facilitate as appropriate.
• Manage expenses, bill hours, and write quality case notes.
• Assist with ISP development and progress reports.
• Provide transportation to participants as needed.
