Job summary

Claro Disability Services is seeking compassionate care professionals to support individuals with complex needs in daily living and mobility. You will work closely with clients, families and clinicians to implement personalized care plans and promote independence.

Responsibilities include physical assistance, monitoring medical plans, facilitating social activities, and advocating for client preferences while maintaining a warm, respectful environment.

• Person-centered planning: chat with client and family/advocates to understand goals and routines.

• Social time and participation: provide meaningful activities and community access aligned with interests.
• Autonomy and advocacy: support decision-making and consent, foster independence.

Shifts available

• Fortnightly Mon 0730-1400
• Fortnightly Fri 0730-1400

What you’ll do (*tailored to each client as required*)

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being
