Responsibilities

• Processing incoming referrals for participation in community programs.
• Data entry and documentation into electronic medical record systems.
• Download/Upload files into various software systems.
• Maintain data spreadsheets and other internal databases.
• Conducting phone calls to schedule in-home assessments for clinical staff and verify timely delivery of services.
• Coordinate service arrangements for program participants and home health providers.
• Perform initial on-location surveys and routine licensing checks for older adult residential settings.
• Responsible for various programs and projects as assigned.
