Job summary

InReach NC is hiring a full-time Support Coordinator (Qualified Professional) in the Charlotte/Mecklenburg area to develop and coordinate Plans of Care for individuals with Intellectual/Developmental Disabilities. You will meet with IDD individuals and families to address needs and collaborate with community partners and InReach staff.

Schedule: Monday–Friday 8:30am to 4:30pm; hours may vary with nights, weekends, and on-call as needed. Location: 4014 Monroe Rd, Charlotte.

Qualifications

• 4-year degree in the human services field.
• 2 years of supervised direct experience with the IDD population.
• 1-2 years of management/supervisory experience preferred.
• Innovations Waiver and 1915i services experience preferred.

Responsibilities

• Supervise Community Outreach staff in implementing supports for individuals served.
• Monitor, manage, and facilitate day-to-day operations.
• Complete all documentation in a timely manner.
• Implement and review each customer’s Plan of Care.
• Plan and oversee work schedules, approve payroll hours.
• Provide one-on-one supervision as needed.
• Share on-call service for emergencies.

Pay Rate: $50,000 - $55,000 based on experience

Responsibilities May Include (but Not Limited To)

Qualifications

• 1-2 years of management/supervisory experience preferred
• Innovations Waiver and 1915i services experience preferred
• Possess a valid driver's license
• Access to reliable transportation 24-hours a day
• Safe driving record.
