POSITION SUMMARY:

The Targeted Prevention Program provides intensive, trauma-informed behavioral health case management and care coordination services to individuals and families who are experiencing or are at risk of homelessness due to eviction or loss of housing. Services utilize evidence-based and research-supported practices designed to promote housing stability, strengthen behavioral health recovery, improve access to community resources, and support long-term self-sufficiency through coordinated, person-centered, recovery-oriented care.

This is a professional supervisory position responsible for the service and administrative oversight of the Targeted Prevention Program within the Housing First Division. The Supervisor of Targeted Prevention provides leadership, coaching, and supervision to Behavioral Health Case Managers and other assigned team members while ensuring high-quality service delivery, achievement of program outcomes, regulatory and contractual compliance, and adherence to evidence-based practices. The Supervisor monitors team member performance, documentation quality, client-facing, client care service expectations, and service provision while promoting trauma-informed, person-centered, recovery-oriented services that improve housing stability, behavioral health recovery, community integration, and long-term self-sufficiency. The Supervisor serves as a subject matter expert in behavioral health case management, housing navigation, community resource coordination, and homeless prevention services, ensuring team members effectively connect individuals and families to behavioral health treatment, housing resources SOAR, Pinellas Health Plan, and other community supports as appropriate. The Supervisor collaborates with internal programs, healthcare providers, housing partners, community agencies, and other stakeholders to ensure coordinated, comprehensive services and is responsible for ensuring the team achieves established client-facing, client care service expectations. The Supervisor is expected to understand the client-facing, client care service requirements for each position and implement strategies that support achievement of those outcomes. Meeting the minimum required weekly, monthly, and annual client-facing, client care service expectations is an essential function of this position. This position requires regular work in community settings.

Directions for Living is an essential services provider. All Directions for Living Team Members must be willing and able to provide vital support and essential services to the people that we serve in the event of an emergency, pandemic or other disaster situation.

QualificationsEDUCATION EXPERIENCE/REQUIREMENTS:
  1. 1. Bachelor’s degree in the Human Services field with a minimum of two years’ leadership experience within an outpatient community mental health organization, homeless services organization or social service agency
  2. 2. Must have or immediately acquire knowledge of all community resources.
  3. 3. Must be computer literate and be proficient in Outlook, Excel spreadsheets, and Microsoft Word. Must be able to work proficiently in a Windows environment and be able to learn other software applications quickly. Must be competent in the use of electronic health records, and be able to use multiple electronic systems including the Pinellas Homeless Information Management System (P-HMIS)
  4. 4. Must have reliable transportation and be able to use personal vehicle to transport clients. Must have and maintain clear driving record, must have valid Florida Driver License, maintain required automobile insurance coverage.
