About This Role

The Supervisor of Care Management oversees the care coordination team serving long-term care members, ensuring quality and efficacy in care management delivery. This role is ideal for individuals with strong leadership skills and experience in care management.

• Pay: $75,300 - $135,400/year
• Location: Remote, IA
• Schedule: Full Time, Day shift
• Service Area: Assigned southwest and south-central Iowa territory
• Work Style: Fully remote

What You'll Do

• Supervise the care coordination team serving long-term care members.
• Monitor and review long-term care management documentation for compliance.
• Assign caseloads and work assignments based on state requirements and member needs.
• Work with senior management on escalated and complex care cases.
• Educate and provide resources for the care management team on key initiatives.
• Evaluate team performance and provide feedback on goals and career milestones.
• Provide coaching and guidance to improve member and provider experience.
• Assist with onboarding, hiring, and training team members.
• Lead and champion change within the scope of responsibility.
• Perform other duties as assigned.

Requirements

Education:

• Bachelor's degree required

Experience:

• 4+ years of related experience required
• For Iowa Only: 4+ years experience with populations served, or RN with 6+ years of experience with the population served

Licenses & Certifications:

• For Iowa Plan Only: Bachelor's degree and 4+ years of experience with populations served, or RN with 6+ years of experience required
