Qualifications

• Driver's License - Illinois Secretary of State (ILSOS).
• Basic Life Support (BLS) within 30 days - American Heart Association (AHA).

Responsabilités

• Identify resources to assist the patient with transportation, medications, and financial barriers.
• Collaborates with behavioral health team to meet needs of high-risk populations.
• Facilitate care transitions and coordinate with other specialties as needed.
• Develops and maintains community relationships to support client referrals.
• Consults with other professionals involved in patient services.
• Attend ROSC-related meetings in or out of the office with prescribers as necessary.
• Document and provide data entry for grant reporting and tracking of services.
• Keep the director informed of problematic cases, including legal or risk management issues.
