Qualifications

• CRSS certification within 2 years as required by IAODAPCA
• Lived experience in recovery is preferred or required
• Advanced reading, writing, and oral English skills required
• Strong communication and interpersonal skills; ability to maintain confidentiality and professional demeanor
• Proficiency with standard computer tools (word processing, spreadsheets, e-mail) and basic EMR/documentation skills

Responsibilities

• Identify patients into services and complete assessment and discharge planning with warm handoff.
• Provide advocacy, education, recovery and wellness support.
• Engage patients in crisis and with therapeutic interventions as needed.
• Identify service needs and apply evidence-based approaches.
• Engage patients in the living room process and coordinate post-discharge support.
• Assist in creating and facilitating recovery planning and discharge planning.
• Ensure patients receive motivation and support to engage in ongoing treatment.
• Document patient interactions in EMR accurately; support quality initiatives.
