Chestnut Health Systems is seeking a full-time Community Health Specialist to support the Bond County Recovery Oriented Systems of Care (ROSC) project. Qualifications

• Bachelor's degree with 5 years of relevant experience, or a master's degree with two years of relevant experience, or be a CPS/CRSS/CPRS/CADC in good standing with ten years' lived experience.
• Knowledge of recovery, program development, needs assessment/evaluation and community capacity building.
• Experience in grant writing and budgeting is preferred.
• Ability to lead, manage large projects, and collaborate with others within the agency and community.
• Advanced skills in Microsoft Office (Word, Excel, Outlook), Zoom and online reporting systems.
• Valid driver's license, private auto insurance, be insurable, and have home internet connection.
• Ability to travel to locations within the site plan area.
• Must be able to obtain DCFS CANTS Child Abuse Clearance.

Responsabilités

• Develop, coordinate, implement and evaluate programs including recovery support events and community trainings.
• Lead community groups in developing evidence-based programs and practices.
• Assist in grant budgeting and submit updates to supervisor.
• Conduct outreach with community stakeholders and support data on partnerships.
• Develop, conduct, and analyze needs assessments and program evaluations.
• Develop, implement, and evaluate media campaigns and educational materials.
• Identify funding opportunities and coordinate grant and contract applications.
• Provide program/grant contract reports timely and accurately.
• Mentor and train staff and interns.
• Participate in local, state, and national advocacy groups/collaborations.
• Fulfill agency and funder responsibilities including staff development and committee participation.
• Promote recovery models and trauma-informed care in all interactions.
• Maintain confidentiality and security of organizational information.
• Promote Chestnut's culture of customer service excellence.
• Perform other duties as assigned.
