Job summary

Carle Health is seeking a SUD ROSC Coordinator to establish a Recovery Oriented Systems of Care in the Peoria area. You will lead development of a local ROSC council and collaborate with community members to align with the IDHS/SUPR mission across prevention, intervention, treatment, and recovery support.

The role requires strong problem-solving, resource knowledge, and advocacy skills to remove barriers to care.

Qualifications

Valid Illinois driver's license (ILSOS).
Basic Life Support (BLS) certification within 30 days (AHA).
Education: relevant healthcare training or equivalent experience.

Responsibilities

Finding resources to assist the patient with transportation, medications, financial barriers, social security or disability claims, or other barriers to safe and successful treatment.
Collaborates with behavioral health team to meet the needs of high risk populations.
Helps to facilitate care transitions and other specialties when needed.
Develops and maintains community relationships to support client referrals.
Consults and collaborates with other professionals involved in providing services to patients.
Attends ROSC-related meetings with psychiatric and medical prescribers as necessary.
Documents and provides data entry for grant reporting and tracking for services delivered through grant funded programs.
Keeps the director/manager informed of problematic cases, especially those involving legal or risk management issues.
