Job summary

North Country Healthcare in Berlin is seeking a SUD Grant and Outreach Specialist (Full Time Hourly) to support the Doorway program and ensure grant compliance for outpatient SUD services in a rural New Hampshire community.

The role collaborates across clinical operations, intake, marketing, finance, and community partners to drive outreach, track outcomes, and support program sustainability. Hourly range listed is $32.41–$43.76, with final pay based on experience.

Qualifications

• Bachelor's degree preferred OR 5 years’ experience in public health, health administration, or related field.
• Certification: BLS/ACLS/PALS or Heartsaver CPR AED, renewal regularly.
• Experience with grant-funded programs (SAMHSA) and Doorway NH system preferred.
• Strong project management, organizational, and documentation skills.
• Excellent written and verbal communication; ability to collaborate across clinical, administrative, marketing, and community stakeholders.
• Knowledge of substance use disorder treatment systems and rural healthcare challenges.

Responsibilities

• Support operational implementation of NH Doorway–aligned, SAMHSA funded activities with approved scopes of work and timelines.
• Assist with grant compliance including documentation, data collection, and progress reporting.
• Track Doorway deliverables such as screening, referrals, and appointment timeliness.
• Serve as a project coordinator for Doorway initiatives, developing work plans and timelines.
• Foster cross-department collaboration across clinical, intake, quality, marketing, and admin teams.
• Coordinate outreach to strengthen Doorway access points, referral pathways, and community awareness.
• Build relationships with regional Doorways, hospitals, and community providers.
• Prepare SAMHSA progress reports and Doorway reporting with the Grant Coordinator.
