Qualifications

• Must be licensed to practice in the state you are providing services in.
• Education includes at least an associate’s degree; a bachelor’s degree is preferred.
• Active LADC licensure in NH is preferred; other credentials may be considered.

Responsibilities

• Conduct focused assessments to identify substance use issues and refer for co-occurring conditions.
• Serve as the primary SUD case manager for each client.
• Develop and implement individualized SUD treatment plans.
• Lead individual and group sessions focused on recovery and coping strategies.
• Provide psychoeducation on substance use disorders and relapse prevention.
• Collaborate with the care team to coordinate services and ensure compliance with guidelines.
• Document all client interactions and progress notes per agency policy.
