Job summary

Qualifications

• Must be licensed to practice in NH; NPI and Medicaid provider number preferred or required to start.
• Associate’s degree in chemical dependency required; Bachelor’s degree in related field preferred.

Responsibilities

• Conduct focused assessments to identify substance use issues and coordinate referrals.
• Serve as primary SUD case manager for assigned clients.
• Develop and implement individualized SUD treatment plans.
• Lead individual and group sessions focusing on SUD recovery using evidence-based curriculum.
• Provide psychoeducation on substance use disorders and relapse prevention.
