Job summary

University Health Partners of Hawai'i seeks a Substance Use Disorder (SUD) Clinical Pharmacy Practitioner to deliver advanced pharmacotherapy management for patients with opioid use disorder under a Collaborative Practice Agreement with a collaborating physician.

The CPP will independently assess patients, order and interpret diagnostic tests, and manage MOUD within an interdisciplinary team, emphasizing harm reduction and shared decision-making.

Qualifications

• PharmD or BPharm from an accredited institution.
• Active Hawaii pharmacist license.
• NPI number must be held.
• Maintains malpractice insurance throughout employment.
• Completes DATA 2000 buprenorphine prescribing requirements.
• Competent in suicide risk assessment tools (PHQ, C-SSRS) and safety planning.
• Competent in advanced physical assessment (COWS, SOWS).
• Minimum two years as practicing clinical pharmacist or completion of residency.
• Preferred PGY1/PGY2 in ambulatory care/psychiatry; board certifications.
• Experience with MOUD and controlled substance inventory in clinical settings.
• Knowledge of pharmacotherapy for OUD and evidence-based care; PDMP data use; strong communication.
• Autonomy to manage a complex patient panel; proficient with EHR; knowledge of Hawaii pharmacy law and DEA rules.
• Experience with motivational interviewing and telehealth is a plus.

Responsibilities

• Provide advanced pharmacotherapy management for OUD under CPA.
• Assess patients independently and order/interpret tests.
• Prescribe MOUD formulations under protocol.
• Coordinate care with interdisciplinary team and ensure harm reduction.
• Manage medication procurement, inventory, and diversion prevention.
• Engage patients and families with shared decision-making.
