Job summary
University Health Partners of Hawai'i seeks a Substance Use Disorder (SUD) Clinical Pharmacy Practitioner to lead evidence-based medication management for patients with opioid use disorder under a formal CPA with a collaborating physician. The CPP provides independent assessment, orders and interprets tests, and makes clinical decisions regarding MOUD.
Beyond clinical duties, the CPP oversees dispensing, procurement, inventory, and diversion prevention within a patient-centered, harm‑reduction
Qualifications
- PharmD or Bachelor of Pharmacy from an accredited institution.
- Active Hawaii pharmacist license.
- Valid NPI number and malpractice insurance.
- Completes DATA 2000 buprenorphine prescribing training.
- Competent in suicide risk assessment and safety planning.
- Knowledge of DEA regulations and Hawaii pharmacy law.
Responsibilities
- Provide evidence-based medication management for OUD under CPA.
- Assess patients, order/interpret tests, initiate and adjust MOUD.
- Manage procurement, inventory, and diversion prevention for SUD pharmacy.
- Collaborate with interdisciplinary team and apply harm reduction principles.
