Job summary

University Health Partners of Hawai'i seeks a Substance Use Disorder Clinical Pharmacy Practitioner (CPP) to lead advanced pharmacotherapy for OUD under a CPA with a collaborating physician. The CPP independently assesses patients, orders tests, and makes clinical decisions including initiation and adjustments of MOUD, writing prescriptions under Order by Protocol.

In addition to clinical duties, the CPP manages SUD pharmacy program operations, handles procurement and inventory, and coordinates

Qualifications

• PharmD or equivalent from an accredited institution.
• Active Hawaii pharmacist license.
• Valid NPI number.
• Maintains malpractice insurance throughout employment.
• Completes DATA 2000 buprenorphine prescribing requirements.
• Demonstrates suicide risk assessment competency and safety planning.
• Demonstrates competency in advanced physical assessment and withdrawal scales.
• Two years of clinical pharmacist experience or residency completed.

Responsibilities

• Provide advanced CMM for OUD under CPA.
• Prescribe controlled substances via Order by Protocol under DEA.
• Manage SUD pharmacy program operations and inventory.
• Engage in care coordination with interdisciplinary team.
• Apply harm reduction, motivational interviewing, and shared decision‑making.
