Job summary

University Health Partners of Hawai'i seeks a Substance Use Disorder Clinical Pharmacy Practitioner to provide advanced medication management for patients with OUD under a formal Collaborative Practice Agreement. The CPP will assess patients independently, order and interpret tests, and manage initiation and titration of MOUD, including buprenorphine.

Qualifications

• PharmD or equivalent credential.
• Active Hawaii pharmacist license.
• NPI number.
• Maintain professional liability insurance.
• DATA 2000 buprenorphine prescribing training.
• Competence in suicide risk assessment and safety planning.
• Advanced physical assessment including COWS/SOWS interpretation.
• Minimum two years as practicing clinical pharmacist or residency completed.

Responsibilities

• Operate under CPA with collaborating physician; clinical independence.
• Provide evidence-based CMM for OUD and related conditions.
• Assess patients independently, order/interpret tests, and make prescribing decisions.
• Write and execute buprenorphine prescriptions under Order by Protocol.
• Oversee SUD pharmacy program operations: procurement, inventory, diversion prevention.
• Be a key member of the interdisciplinary care team; provide care coordination.
• Apply harm reduction, motivational interviewing, and shared decision-making principles.
• Ensure compliance with Hawaii law and DEA regulations.
