Job summary

Goochland Powhatan Community Services is seeking a Full-Time SUD Care Coordinator to join our team. This role focuses on case management for adolescents and adults with substance use disorders, including OBAT team involvement and client advocacy.

The ideal candidate will have a Bachelor’s in Human Services and CSAC-S preferred, with 1–2 years in case management. A Virginia driver’s license and background check are required.

Qualifications

• Bachelor’s degree in Human Services or related field required.
• CSAC-S credential preferred.
• 1–2 years of case management/care coordination experience with Opiate Use and other SUDs encouraged.

Responsibilities

PRIMARY RESPONSIBILITIES

SUD care coordinator provides case management services to adolescents and adults living with substance use disorders. Case management includes connecting the client with services, advocacy, and providing supportive counseling to aid with coping with substance use disorders, with special focus on individuals with Opioid use disorders on our OBAT team. SUD Care coordinator will document visits and supports within agency and state guidelines.

MINIMUM JOB REQUIREMENTS:

Education

Bachelor’s Degree in Human Services field of study (example include Social Work, Counseling or Psychology) and/or CSAC-S. CSAC preferred.

Experience

One to two years related experience in case management/care coordination working with individuals with Opiate Use and other Substance Use Disorders. Experience necessary for credentialing as a Qualified Mental Health Provider may be considered.
