Goochland Powhatan Community Services Board in Goochland, VA is seeking a Full-Time SUD Care Coordinator to provide comprehensive case management for adolescents and adults with substance use disorders, including advocacy and supportive counseling on the OBAT team.

Qualified candidates will have a Bachelor's degree in Human Services (CSAC-S preferred), 1–2 years of related experience, and a Virginia driver's license; background check is required.

Formación

• Bachelor's degree in Human Services or related field; CSAC-S preferred.
• 1–2 years case management/care coordination experience with substance use disorders.
• Virginia driver's license and ability to pass background check.

Responsabilidades

• Provide case management for adolescents and adults with substance use disorders.
• Connect clients with services, advocate for them, and provide supportive counseling.
• Document visits and maintain state and agency guidelines; focus on OBAT team.
