Job summary

Harbor is seeking a Success Coach to join the Ohio Means Jobs Lucas County team in Toledo, OH. The role delivers professional support services to participants and families enrolled in the Benefits Bridge Program, drawing on lived experience to guide clients toward self-sufficiency.

You'll conduct assessments, develop service plans, connect clients to community resources, and support them in navigating benefit cliffs while coordinating with Care Teams and partners.

Qualifications

• Experience delivering services to participants and families in public assistance.
• Proficient in computer use, including Microsoft Word.
• Must obtain EMPath Certificate within 120 days of hire.
• Valid driver’s license with insurable record.

Responsibilities

• Uses experiences with public assistance to support participants ongoingly.
• Completes assessments, bridge scores, and develops service plans.
• Refers clients to financial literacy training and related programs.
• Provides direct services, maintains weekly contact, monitors goals and incentives.
• Assists in documenting eligibility for community resources.
• Acts as advocate to secure needed services and coordinates care team updates.
• Links participants to financial literacy trainings and supports community services.
