Job summary

Harbor is seeking a Success Coach to join the Ohio Means Jobs team in Toledo. The role supports program participants and families in the Benefits Bridge program, leveraging lived experience and strong community ties.

Position is full time, first shift, 40 hours weekly. Requires a high school diploma or GED; associate’s degree preferred; EMPath certificate within 120 days; valid driver's license; and availability for nights/weekends as needed. Proficiency in MS Word and driving are essential.

Qualifications

• High School Diploma or GED required; associate’s degree or higher preferred.
• Experience receiving public assistance is advantageous.
• Must obtain EMPath Certificate within 120 days of hire and maintain CE requirements.
• Must be able to work nights and weekends when needed.
• Proficiency with Microsoft Word and basic computer skills.

Responsibilities

• Complete assessments, bridge scores, and develop service plans with participants.
• Refer and support participants in attending financial literacy trainings.
• Provide direct services, maintain weekly contact, and monitor progress toward goals.
• Assist in documentation to determine eligibility for community resources.
• Advocate for participants to access needed services and attend Care Team meetings.
• Coordinate with internal/external providers and support self-sufficiency goals.
