Qualifications

• Master's degree in social work or related field required.
• CASAC certification required or preferred.
• Strong written and verbal communication and documentation skills.

Responsibilities

• Conduct assessments and develop individualized service plans for co-occurring disorders.
• Provide individual and group substance use counseling and psychoeducation.
• Coordinate screenings and referrals to treatment providers and resources.
• Collaborate with ACT Team to integrate treatment goals into plans.
• Maintain accurate EHR documentation and ensure regulatory compliance.
