Job summary

Access: Supports for Living is seeking a Substance Use Specialist to join our ACT Team in Valley Cottage, NY. You will provide substance use counseling, education, and support to individuals with co-occurring mental health and substance use challenges.

The role involves maintaining a caseload, conducting intake interviews, and coordinating care with an interdisciplinary team using harm reduction, trauma-informed, and recovery-oriented approaches to help participants achieve recovery and life

Qualifications

• Excellent communication, writing, and organizational skills.
• Knowledge of co-occurring disorders, evidence-based practices, and recovery supports.
• Ability to use electronic health record systems for documentation and communication.

Responsibilities

• Maintain a caseload of 12-15 participants and provide case management services.
• Perform intake interviews and evaluate appropriateness for treatment.
• Complete assessments and treatment plans within required timeframes.
• Provide crisis intervention and participate in on-call rotation.
• Coordinate care with interdisciplinary team and community partners.
