Job summary

Crossroads Treatment Centers is seeking a Substance Abuse Counselor to provide individual counseling to complex patients as part of a comprehensive MAT program. You will conduct assessments, design and update treatment plans with referrals and discharge planning, and manage a caseload while ensuring compliance with clinical requirements.

Collaborating with nurses and providers, you will help deliver holistic care and support patients on their path to recovery in an outpatient setting.

Qualifications

• General experience working with underserved populations in a clinical setting.
• Lived addiction experience is a plus.

Responsibilities

• Provides individual counseling to highly complex patients.
• Conducts clinical assessments to appropriately assess patient’s treatment needs.
• Designs and updates the patient’s treatment plan, including care coordination, referrals, and discharge planning.
• Manages assigned caseload and compliance with clinical treatment requirements.
• Collaborates with nurses and providers to provide holistic treatment services.
