Job summary

Crossroads Treatment Centers is seeking a Substance Abuse Counselor to join our outpatient team. You will provide individual counseling to highly complex patients and help design and update their treatment plans within a MAT framework.

Responsibilities include conducting assessments, coordinating care with nurses and providers, and overseeing a targeted caseload. A strong commitment to serving underserved populations is essential.

Qualifications

• General experience working with underserved populations in a clinical setting.
• Lived addiction experience is a plus.
• Candidates should hold LMSW, LCSW, LPC or LADC certifications/licenses.

Responsibilities

• Provides individual counseling to highly complex patients.
• Conducts clinical assessments to determine treatment needs.
• Designs and updates the patient’s treatment plan, including care coordination, referrals, and discharge planning.
• Manages assigned caseload and ensures compliance with clinical treatment requirements.
• Collaborates with nurses and providers to deliver holistic treatment services.
• Facilitates urine drug screening for patients.
