Zusammenfassung

Crossroads Treatment Centers is seeking a Substance Abuse Counselor to provide individual counseling to highly complex patients and conduct clinical assessments to determine treatment needs, while designing and updating personalized treatment plans within an evidence-based MAT framework.

You will manage a caseload, coordinate with nurses and providers, and facilitate urine drug screening, participating in a rotating Saturday schedule as needed.

Qualifikationen

• General experience working with underserved populations in a clinical setting.
• Lived addiction experience is a plus.
• Qualified candidates will hold LMSW, LCSW, LAPC, LPC, or LADAC licensure.

Aufgaben

• Provides individual counseling to highly complex patients
• Conducts clinical assessments to appropriately assess patient's treatment needs
• Designs and updates the patient's treatment plan, including care coordination, referrals, and discharge planning
• Manages assigned caseload and compliance with clinical treatment requirements
• Collaborates with nurses and providers to provide holistic treatment services
• Facilitates urine drug screening for patients
