Job summary

Crossroads Treatment Centers is seeking a Substance Abuse Counselor to join our clinical team. You will provide individual counseling to highly complex patients, perform clinical assessments, and design/update individualized treatment plans with care coordination and discharge planning.

You will manage caseloads, ensure adherence to treatment requirements, collaborate with nurses and providers, and facilitate urine drug screening.

Qualifications

• Experience with underserved populations in a clinical setting.
• Lived addiction experience is a plus.
• Licensure: LMSW/LCSW/LPC/LADAC or equivalent.

Responsibilities

• Provides individual counseling to highly complex patients.
• Conducts clinical assessments to determine treatment needs.
• Designs and updates the patient’s treatment plan, including care coordination, referrals, and discharge planning.
• Manages assigned caseload and compliance with clinical treatment requirements.
• Collaborates with nurses and providers to provide holistic treatment services.
• Facilitates urine drug screening for patients.
