Job summary

Crossroads Treatment Centers is seeking a Substance Abuse Counselor to provide direct counseling for patients with opioid use disorder at our outpatient clinics. You will conduct assessments, design and update treatment plans, and coordinate care with nurses and other providers to ensure holistic treatment.

The role emphasizes managing a defined caseload, adhering to clinical requirements, and participating in Saturday rotations as part of a consistent schedule.

Qualifications

• Bachelor's Degree preferred but not required.
• CADC, Level I; CADC II; CAADC; CAC I; CAC II; MAC; NCAC I; NCAC II; LPC/LAPC/LMSW endorsements.

Responsibilities

• Provides individual counseling to highly complex patients.
• Conducts clinical assessments to appropriately assess patient’s treatment needs.
• Designs and updates the patient’s treatment plan, including care coordination, referrals, and discharge planning.
• Manages assigned caseload and compliance with clinical treatment requirements.
• Collaborates with nurses and providers to provide holistic treatment services.
• Facilitates urine drug screening for patients.
