Job summary

Lund's Regional Partnership Program (RPP) is hiring a Case Manager to support families affected by parental substance use, working across Barre and Hartford DCF offices. The role screens for substance use, links parents to treatment, and helps remove barriers to engagement in multiple community settings.

The starting rate is $25/hour. Applicants should have an associate degree and experience in case management within related settings; AAP certification paths are supported by Lund.

Qualifications

• Minimum of an associate degree required; bachelor's degree in social work, counseling or human services preferred.
• Experience providing case management in substance use, recovery, or mental health settings preferred.
• AAP certificate or ability to test for certification within first year; Lund provides support to obtain the certificate.
• Experience with families, multidisciplinary teams, and knowledge of community resources preferred.
• Ability to maintain client confidentiality according to state and federal regulations.
• Proficiency with Microsoft Office, Electronic Health Records, and video conferencing (Zoom/Teams).

Responsibilities

• Screen for problematic substance use and provide early intervention at the front end of child protection.
• Coordinate referrals to assessment and treatment providers and address barriers to engagement such as transportation and childcare.
• Collaborate with DCF staff and liaise between the treatment system and DCF Family Services.
• Deliver services in DCF offices, community spaces, and clients' homes to meet families where they are.
