Job summary

Lund's Regional Partnership Program (RPP) seeks a dedicated Case Manager to support families, address substance use challenges, and improve well-being and permanency outcomes in Vermont communities. The role collaborates with DCF staff to screen for issues and connect parents to treatment while removing barriers to engagement.

Based at the Springfield DCF district office, the role requires a valid VT driver's license and willingness to travel.

Qualifications

• Associate degree required.
• Bachelor's degree preferred in social work, counseling or human services.
• Experience in substance use, recovery, or mental health case management preferred.

Responsibilities

• Screen for substance use concerns and coordinate referrals to assessment and treatment.
• Link families to appropriate treatment services and overcome engagement barriers.
• Provide consultation and training to DCF staff on substance use and treatment services.
• Work across multiple settings (offices, community spaces, clients' homes) to support families.
