Job summary

Telecare is seeking a Case Manager Counselor (CADC/CATC/SUDCC) for the Gateway to Recovery ACT in San Diego. The role focuses on supporting individuals with mental illness and substance use disorders, promoting independence, and coordinating community-based services.

The position is full-time, on-site from 8:00 AM to 5:00 PM, Monday through Friday, with a starting wage range of $26.46–$32.69 per hour. Mileage assistance and comprehensive benefits are included.

Qualifications

• High School Diploma or GED and CADC/CATC/SUDCC or equivalent certifications.
• Six months of experience in addictions setting.
• Certifications must be maintained for duration of employment.
• Access to billing knowledge within 2 months of employment.

Responsibilities

• Serve as primary support and case manager for individuals with mental illness focusing on substance use disorders.
• Assist clients to live independently in their community.
• Coordinate care and services across providers.
