• Supports patients with substance use disorders through education and linkage to services.
• Performs education, service coordination, and care navigation across healthcare, social services and justice systems.
• Builds trust-based relationships to encourage engagement in treatment for optimal outcomes.

Responsibilities

• Provide ongoing care-coordination and case management.
• Guide linkage to clinical services including MAT, behavioral health treatment, and social services.
• Provide education, coordination, navigation, linkages to individuals with Substance Use Disorder.
• Foster continuity of care by meeting patients in various clinical care settings.
• Attend Addiction Medicine clinical and programmatic conferences and meetings as required.
