Job summary

Cooper Norcross Health in Camden, NJ seeks a Community Outreach Coordinator to lead field-based outreach and care navigation with the MIH team. You will engage individuals living with substance use disorders across the City of Camden, coordinating with an MIH Clinician on non-emergent overdose responses and community outreach activities.

The role requires 3–5 years in public health or community-based work, strong communication, and proficiency with Microsoft Office; EHR experience is helpful.

Qualifications

• 3–5 years in community-based project planning or public health programs.
• Ability to manage stakeholders and provide direct patient services.
• Experience in community outreach/education programs.

Responsibilities

• Coordinate and facilitate field-based outreach and care navigation with the MIH team.
• Accompany the MIH Clinician in an MIH vehicle to respond to overdose-related calls and conduct outreach.
• Engage individuals through multiple pathways to care (overdose responses, hospital discharges, referrals).
• Document patient encounters, referrals, and outcomes in the EHR and other data systems.
• Track and report outreach activities and program outcomes for grant reporting and quality improvement.
