Job summary

This planned, part-time position begins once approvals and staffing readiness are in place. Pay ranges from $24–$32 per hour, depending on qualifications and experience, with on-site or community assignments as applicable.

• Relevant behavioral-health education and care-management experience.
• Required program credentials and supervision verified before service delivery.

Responsibilities

• Coordinate substance use care plans, treatment connections, follow-up and communication across providers; document care management within the eligible parent program.
• Work collaboratively with department leadership, intake, credentialing, billing and quality teams.
• Maintain confidentiality, respect participant choice, communicate service gaps promptly, and follow role-specific policies.
• Participate in orientation, competency review and quality improvement.
