Job summary

Grace Community Care and Homes Inc. is expanding its Addiction Treatment and Recovery services and seeks a Substance Use Care Manager. The role emphasizes person-centered care, reliable operations and accurate documentation across care teams.

The position is part-time with start date contingent on approvals and staffing; work is conducted on-site in New Jersey or community-based assignments, with hours discussed during hire and pay ranging $24–$32 per hour based on qualifications and experience.

Qualifications

• Relevant behavioral-health education and care-management experience; required program credentials and supervision verified before service delivery.
• Final regulatory eligibility will be verified before appointment or service delivery.

Responsibilities

• Coordinate substance use care plans, treatment connections, follow-up and communication across providers; document care management within the eligible parent program.
• Work collaboratively with department leadership, intake, credentialing, billing and quality teams.
• Maintain confidentiality, respect participant choice, communicate service gaps promptly, and follow role-specific policies and professional scope.
• Participate in orientation, competency review and quality improvement.
